Endoscopic Biliary Drainage of Hilar Cholangiocarcinoma
نویسنده
چکیده
Cholangiocarcinoma is an uncommon malignancy which arises from biliary epithelial cells, anywhere along intraand extrahepatic biliary tract and can be divided into three groups by involved anatomy: intrahepatic, perihilar, and distal extrahepatic cholangiocarcinoma. Cure can only be achieved by surgical resection. Because of initial diagnosis as advanced disease and recurrence despite surgery and resistance to chemotherapy or radiotherapy, overall prognosis is very poor. Hilar cholangiocarcinoma (HC), also well known as Klatskin tumor, involves bile duct bifurcation area and/or both the right and left intrahepatic bile ducts, so it usually causes biliary obstruction at presentation. In the situations, biliary decompression is mandatory to relieve cholestatic symptoms, if the patients are not suitable for surgical resection because of advanced tumor stages or comorbidities. Here, it will be reviewed about endoscopic biliary drainage of HC.

Endoscopic drainage in patients with inoperable hilar cholangiocarcinoma
Hilar cholangiocarcinoma has an extremely poor prognosis and is usually diagnosed at an advanced stage. Palliative management plays an important role in the treatment of patients with inoperable hilar cholangiocarcinoma. Surgical, percutaneous, and endoscopic biliary drainage are three modalities available to resolve obstructive jaundice. Plastic stents were widely used in the past; however, se...

Background: Most cholangiocarcinoma patients present at an advanced stage of the disease. Palliative treatment options include PTBD and ERCP with biliary stenting. Although quality of life (QOL) is of importantce in terminal or advanced malignancy, a number of studies on palliative ERCP and biliary stenting focus mostly on stent patency and survival, with limited data on QOL outcomes. The prese...

Endoscopic palliative biliary drainage is considered as a gold standard treatment in advanced or inoperable hilar cholangiocarcinoma. Also, metal stents are preferred over plastic stents in patients with >3 months life expectancy. However, the endoscopic intervention of advanced hilar obstruction is often more challenging and complex than that of distal malignant biliary obstructions. In this l...
متن کاملCurrent Status of Endoscopic Treatment of Advanced Hilar Cholangiocarcinoma
Cholangiocarcinoma is a high-mortality primary hepatic malignancy. A higher incidence of cholangiocarcinoma was reported in Asia, especially Southeast Asia, than in Western countries. Hilar cholangiocarcinoma is a specific type of extrahepatic cholangiocarcinoma that involves the hepatic hilum and has a worse prognosis. More than half of the patients with jaundice are inoperable at the time of ...

In patients with inoperable hilar cholangiocarcinoma (HCCA), palliative endoscopic or percutaneous drainage provides benefits in terms of symptomatic improvement and quality of life. Endoscopic biliary stent placement is considered the gold standard, with metal stents preferred over plastic stents in patients with more than three months of life expectancy. However, the endoscopic management of ...
